Welcome back to SMS. I am looking forward to another HEALTHY year here in the nurse's office. My office will be open from 9:00am to 2:00pm daily. If the teacher sends your ill student to the nurse’s office before 9am, you will be notified to pick him or her up in the school office. If your student feels ill or has a fever, as defined in the school handbook, please keep him or her home that day. Please make sure that your student gets plenty of rest and eats a healthy breakfast every day for a good start to the new school year. All medical forms must be in the nurse’s office by the 10th day of school or your student may not return to the classroom. Indiana State Law requires documented proof of required immunizations to be in a student’s health file. Public Law 150, effective August 1, 1980, requires immunization of all children enrolled in schools. This law was amended July 2, 1985, requiring that a student shall not attend beyond the first day of school without one of the following: -Current immunization record provided to the school - Statement from physician that required immunizations were delayed by extreme circumstances and a time schedule for immunization has been established -Written statement of health, religious or other objections provided to school. Written proof of immunizations may be obtained from a family doctor or the Lake County Health Department. Students will not be permitted to attend school beyond the first day of school without a written statement unless a waiver is granted by the school. The waiver shall be granted for no longer than 20 days. Information regarding Meningococcal Disease/Vaccines (as required by Indiana State Law (IC 20-30-5-18) will be in the first Wildcat along with information on West Nile Virus. Information regarding the Hepatitis B Vaccinations was sent home in the 2007-2008 school year packet and given to all new students. If you have any questions, please contact your health provider for specific instructions regarding your student. Please avoid sending medication to school if at all possible. For example, medicine taken three times a day may be given before school, after school and at bedtime. Also, over-the-counter medicine such as sinus or allergy medicine can be taken in the morning before school. If it is necessary to bring medicine to school for the nurse to administer, the following should be observed as defined in State Law I.C . 34-30-4, 34-30-14-2(2), 34-30-14-2. The medication needs to be in its original container and labeled. It must be accompanied by a parent and doctor's note. This note should include the student's name, medication and dosage instructions. Remember, this applies to over-the-counter medicine as well, no exceptions. It is the child’s responsibility to come to the office when it is time for medication to be given. A parent or authorized person may always come to school and personally give medication to their child as long as the nurse or office personnel are made aware of this.
